How to select the right Case Manager

Selecting the right case manager is crucial to ensuring P receives the best possible care and support. Here’s a step-by-step guide to help you make an informed decision:

1. Understand P’s Needs

  • Type of Case Management: Identify the specific type of case management P needs (e.g., healthcare, social services, mental health, disability support).
  • Specific Requirements: Consider any specific conditions, disabilities, or needs that require specialized knowledge.

2. Research and Referrals

  • Professional Recommendations: Ask for referrals from healthcare providers, social workers, or other professionals.
  • Company Referrals: Seek recommendations from Circle Case Management; after triage suitable Case Managers C.V’s will be put forward to you.
  • Professional Bodies: Check professional bodies such as the British Association of Brain Injury and Complex Case Management (BABICM) or the Case Management Society UK (CMSUK).

3. Check Qualifications and Experience

  • Qualifications: Ensure the case manager has relevant qualifications and certifications.
  • Experience: Look for someone with experience in managing cases similar to P’s.
  • Specialisation: Confirm if they have expertise in the specific area of need (e.g., brain injury, spinal injury, mental health).

4. Interview Potential Case Managers

  • Initial Consultation: Schedule an initial consultation to discuss P’s needs and assess their suitability. Circle Case Management will provide a free meet and greet for P.
  • Approach and Philosophy: Understand their approach to case management and how they plan to support P.
  • Communication: Ensure they have good communication skills and that P feels comfortable discussing sensitive issues with them.

5. Assess Compatibility

  • Personal Connection: It’s important to feel a personal connection and trust with your case manager. Circle Case Management will always recommend Case Managers based on their suitability of experience and expertise, as well as considering a suitable match in personalities.
  • Availability: Check their availability to ensure they can provide the support needed within P’s required timeframe. Circle Case Management will only recommend Case Managers with capacity – all Case Managers are case load managed. All Case Managers are supported by a 2nd Case Manager, Assistant Case Manager and Head Office – ensuring annual leave, sickness or any other leave does not have any adverse effect on P or communication with instructor.

  • Professional Memberships: Confirm their membership in relevant professional organizations.
  • Background Check: Ensure they have a clear professional record and no history of complaints or disciplinary actions.
