Child with Cerebral Palsy

In 2016 Circle were asked to work with a young boy and his family. Following an initial case management report initial goals were agreed which included; the recruitment of a full direct employed support worker team, completion of an education, care and health plan, starting school, producing a care plan, obtaining an OT assessment for eating and drinking, obtaining a speech and language assessment, obtaining equipment highlighted by the occupational therapist, reviewing current therapy input, completion of risk assessments, liaison with family support services and social services and ensuring adequate transport provision. The family at that point were not engaging with statutory services. The Case Manager set to work and through trust, communication and professional excellence was able to re-engage the family for the benefit of her client and build a team of professionals around the client. Ultimately this has led to the client making significant progress in all areas towards a full and active life. The client is at school 3 days a week, is safe, physically stronger and moving forward in his young life.
